1.  Pediatrician Referral that reads "Evaluate and Treat Speech Therapy" with your child's diagnostic codes (fax to 678-819-2810)
2.  Specialist reports (If applicable)- Reports from Neurologist, Developmental Pediatrician/Psychologist, GI Specialist, Swallow Study, etc
3.  Hearing Screening/Evaluation (with results in Hertz) required if your child has PeachState or Amerigroup
